The Title is "The Art of Star Trek" and while I am not sure what William Ware Theiss was thinking you can see on pages 88-91 it could have been allot worse. Author Judith and Garfield Reeves-Stevens with an introduction by Herman Zimmerman ©1995 Paramount Pictures (The pages may be different but I doubt they would change the layout much in other copies.) I have a book you might be able to find at a nearby library that may shed a little light on this fashion disaster.
